Assisted Living in Arizona

"My mother has been in the memory care unit Revere for approximately 3.5 years. Recently my mom took a fall out of her wheelchair chair about a month and a half ago She hit her head and has a hairline fracture on her ankle. She has been transported to her doctors appointments at least 2 times by the facility and once by private medical because they were booked. She is currently 91. Recently I was told that I needed to pay $40/hour for a caregiver to go with her on the bus. I meet my mom at her doctor appointments and follow up with the facility for doctors orders afterwards. A caregiver went on her second trip but the last two visits nobody ride with her. I have requested a meeting but as of today no response. I am highly disappointed." - D… Read more

"Don't be fooled by appearances. I cared for my mother in my own home for 5 years. When it got to the point where I couldn't care for her and work a full time job, I thought this place looked lovely and everyone seemed so caring. She ended up passing away after two weeks. The total bill for those two weeks ended up coming to over $11,000.00. This amount seems excessive for two weeks of stay doesn't it? This would include paying for an additional 4 wk.s of a room even though she wasn't in the room. This was contractual but I was told by admin that "this didn't seem right" since she was only there two weeks, which I agreed. They requested with the VPO to reimburse for part or all of that amount, that was declined. The only way I found out was by repeatedly calling the Executive Director that only returned my call after leaving four messages directly to her over a week and a half and then claiming that we hadn't left three of those messages. "My parents moved to the citadel 5 plus years ago. It was a wonderful place then. The people were happy. It was clean, (although the wallpaper, carpet and furniture needed to be replaced). The staff and Executive Director were interactive with the residents and knew their names. The food was great. There were many options to choose from and the portions size was good. There were at least 2 medtechs on each floor along with 4 or more caregivers to take care of the residents each shift. The communication between the Executive Director and the staff was great due to the willingness to listen to one another and work together to make things run smoothly and at its best. The Executive Director knew everyone, but what made him stand out was his willingness to go the extra mile to help others. By others, I mean the residents and the staff. For instance when the kitchen staff was short handed he got in, took orders, cleaned off tables, and did what needed doing. He also would Executive Director worked side by side with care taker helping a resident & empty trash, etc. This Director had good people and leadership skills. Things have changed. There've been 5+ different directors/managers. Concerns from residents, relatives of residents & staff to those in management positions fall on deaf ears. Food options and portions are much smaller. MANGEMENT did away with security guard & trimmed down staff: 1 cook and 2 servers for 100+people, caretakers & medtechs-1 each on bottom floor, 1or2 mdtk & 1or2 caretakers top floor. MEDTECH DUTIES: intake of meds from provider, organize & distribute correct meds to residents, Help caregivers, mandatory paperwork! CAREGIVER DUTIES: Baths, falls, Bathroom help, poop/pee & vomit clean up, take residents back & forth to meals/activities, answer residents call, pickup trash. "Our friend Jackie had an old fashioned Tea Party in the Presidents Dining room on Friday. Jackie and my wife set the room up so beautiful with the Help of Sabrina and here staff. They set up extra tables chair supplied coffee ice water for the 24 Ladies attending. Jackie had the party catered with finger foods and great pastries. I did a small tour of the Park at Surprise what a great place to visit and maybe stay in a latter time.
