For 20+ years, we have conducted care evaluations and helped families navigate care options. Our CareScout Quality Network is the first long-term care network where providers meet rigorous quality standards and offer preferred pricing. 
And our personalized Care Plans, created just for you by a licensed nurse, help you take the guesswork out of what type of aging care, equipment, and support may be appropriate for you or your loved one.

How long will the care evaluation take?

A care evaluation typically will take about 45-60 minutes.

When will I receive my Care Plan?

Accepted Answer

Your Care Plan will arrive within five business days after your care evaluation.

Question 4

Can I be with my loved one for the care evaluation?

Accepted Answer

Yes. If your evaluation takes place in person, you may choose to be in person with your loved one during the evaluation. If you choose a virtual evaluation, your nurse will send a Zoom link. You can use that link to log in individually from separate locations or to log in together from the same location. However, if you choose an in-person evaluation, you will not be able to join by Zoom from another location.

What should I expect during the evaluation?

The evaluation will start with the nurse gathering some basic information. From there, the nurse will walk through each of the activities of daily living (ADLs) and instrumental activities of daily living (IADLs), and ask you to talk about how you perform them. 
They’ll learn about your lifestyle, home, diet, social connections, and care network. And they’ll check whether you have certain important legal documents. 
Finally, there’s a cognitive section where you’ll answer questions to assess orientation, recall, registration, and attention. You’ll also discuss your moods and cognitive history.

How thorough are the evaluations?

The nurses who comprise our Field Team have experience completing both in-person and virtual evaluations. Whether conducted virtually or in-person, our care evaluations are a hands-off experience, meaning they do not require physical interaction or the taking of medical readings. They will simply ask relevant questions to understand the level and type of care that may be beneficial to you.
 
In-Home Evaluations
The nurse will ask if they can view your primary living spaces. As part of the in-home hazard and personal safety review, the nurse will evaluate the environment for potential risks such as tripping hazards, inadequate lighting, and other safety issues that might affect your well-being. Their home safety recommendations will be included in your Care Plan.

How soon can I get an evaluation?

When you schedule your evaluation, you will be asked to provide three possible dates and times that might work for you, and we will find a nurse that can meet with you within those days and times. When scheduling, you can provide times as early as three days out or as distant as two months out.

How do I receive my Care Plan?

Your Care Plan will be emailed to you as a printable PDF.

What exactly is in my Care Plan?

Accepted Answer

Your Care Plan is a thorough report detailing what level and type of care, support, and resources you may need based on your responses and the nurse’s observations. It will include five sections: self-care, home safety, medical management, lifestyle and emotional support, and advance care planning.
For each section, you’ll find a summary of your nurse’s observations, suggestions for care or supports that can help you in those areas, changes to look out for, and resources to help you learn more or take next steps. It is personalized and actionable, giving you information relevant to your particular circumstances and in your area. 
This is not a medical service, and we do not provide medical advice. Our goal is to help you enjoy a dignified and fulfilling aging experience by outlining non-medical care, supports, equipment, and resources that can help you keep being you.

Question 10

If I decide I need home care, will my provider want to do another assessment? If so, is there a separate fee and what's the difference between a provider assessment and a Care Plan?

Our care evaluation is designed to help us develop a Care Plan that covers a broad set of issues, including issues beyond home care needs, and offers education and next steps for you to consider. A provider assessment will address more specific details that help them determine the best caregiver for your needs and to set that caregiver up to meet those needs. A provider might charge an additonal fee for their assessment.

Is a Care Plan covered by insurance?

No, our Care Plans are not covered by any type of insurance.

What’s the difference between Long-Term Care Insurance and a Care Plan?

Accepted Answer

A Care Plan is not for the purpose of obtaining benefits under a Long-Term Care Insurance (LTCi) policy. For example, a Care Plan is not an alternative to a benefit eligibility assessment, Chronically Ill Certification or Plan of Care, as may be required by your LTCi policy.
Some LTCi policies may also provide assessments and other care-related information benefits at no cost to you.
If you have an LTCi policy and think you may be eligible for benefits, or have questions about your LTCi coverage, please contact your insurance carrier directly.
