The terms of use for the CareScout, LLC (“CareScout”) webpages, including but not limited to CareScout.com and the related information (including, but not limited to, educational articles and tools), and other services from CareScout (the “Services”), are set forth below (the “Agreement”).

This Agreement contains important disclaimers.

Please read this Agreement carefully before using or continuing to use the Services. By accessing, using, or interacting with the Services, you agree to be bound by this Agreement. The Services are provided by CareScout. Unless stated otherwise, any new services and materials added to this website will also be covered by this Agreement.

 

Use of Services

The Services are intended solely as a general educational aid and to make you aware of current and upcoming offerings from CareScout, including the CareScout Quality Network and opportunities for Providers or the CareScout Field Team.

The Services are provided with the understanding that CareScout is not engaged in providing legal, medical, or counseling advice or other similar professional services. Your use of the website is subject to the additional disclaimers and caveats that may appear throughout the website.

 

Definitions

The term “Provider” as used in this Agreement shall mean facilities, home care providers, and other similar businesses.

The term “CareScout Field Team” as used in this Agreement shall mean those agencies and individuals (also referred to as CareScout Field Partners) who are independently contracted with CareScout to provide its assessment services.

 

Information Provided in Connection with the Services

The information provided in connection with the Services is intended to serve as a guide and basis for general information, but not as the sole basis upon which any material decision should be made.

You are advised that CareScout is not responsible for the accuracy of the information or for any errors, omissions, or the application of this information or for any consequences arising therefrom. Therefore, CareScout shall have no liability to any person or entity with regard to claims, loss, or damage caused or alleged to be caused, directly or indirectly, by the use of information contained herein.

ALL INFORMATION AND CONTENT ON CARESCOUT WEBSITES ARE FURNISHED "AS IS," WITHOUT WARRANTY OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NONINFRINGEMENT.

CareScout uses your Personal Information in accordance with its Privacy Policy. Personal Information refers to any information that can be used to reasonably identify you, such as your name, phone number, or email address. Please click here  to review the Privacy Policy.

 

CareScout Quality Network  

CareScout Services:

CareScout can help you find providers to meet your long-term care needs. Providers in CareScout’s Quality Network have agreed to preferred pricing, meet CareScout’s quality standards and are committed to person-centered care. Providers outside the CareScout Quality Network may provide same or similar levels, types, and quality of services, though any information CareScout has on them is based on public sources and has not been reviewed by CareScout.

Purpose:

When search results are provided to you, they are meant to assist you in identifying a Provider you may be able to use for your long-term care needs. Search results are not being provided to help you identify a Provider of services covered by Medicare, Medicaid, or other federal or state health care programs, and you don’t have to use any Provider identified by CareScout, including the Providers in the CareScout Quality Network, in order to access benefits under a government health care program such as Medicare or Medicaid or to access benefits under a long-term care insurance policy (“policy”). In addition, you are responsible for confirming the eligibility of any Provider under any long-term insurance care policy you may own. Any preferred pricing available through the CareScout Quality Network will not apply to services or any portion of services covered by a federal or state health care program.

No obligation: You can use any eligible Provider for policy coverage:

You are under no obligation to use CareScout to locate a Provider or to use any Provider CareScout identifies. To receive coverage under your policy, you can use any Provider whose services are eligible for policy coverage as long as you also meet all other coverage requirements. You can determine whether the services of a Provider, including a Provider CareScout identifies or that you identify elsewhere, are eligible for policy coverage by contacting your insurance company.

 

How CareScout selects the list of Providers:

CareScout’s goal is to identify and share a list of Providers in your area that could meet your needs. CareScout generally identifies Providers based on the selection criteria you share with us, availability, their proximity to your zip code, and whether the Provider is in the CareScout Quality Network. It is important to understand that the search results received from the Services may not be the lowest cost Providers in your area. 

When possible, CareScout seeks to provide a choice of Providers both in and outside the CareScout Quality Network. If the initial results driven by your search criteria do not contain any Providers outside the CareScout Quality Network, you can expand search results to include one or more of these Providers, if any, that match your criteria.

If you have questions about how Providers in the search results were selected, or to inquire about additional Providers in your area, call CareScout at (800) 543-2901 Monday-Friday from 8:00 am – 6:00 pm EST. In addition to the search results provided to you by CareScout, you may also find Provider information from other sources, including, for example, community resources on aging or, if you are being discharged from the hospital, the hospital’s discharge planner.

Providers are solely responsible for the care and services they provide:

Neither CareScout nor your insurance company are providing long-term care services to you and neither are responsible for the delivery of care or actions of any Provider you select. Any services you receive from a Provider are solely the responsibility of that Provider.

Using a Provider CareScout identifies does not guarantee policy coverage:

Using CareScout to help you locate a Provider does not guarantee coverage under the terms of your policy, policy coverage for the services of a Provider, or approval of a claim for policy benefits. A claim determination can only be made by your insurance company once a claim has been filed and all requirements for payment of the claim have been met. If you have questions about insurance coverage, claim eligibility, or whether the services of a Provider, including a Provider identified by CareScout or that you identify elsewhere, are eligible for policy coverage, please contact your insurance company.

 

Preferred pricing on Provider rates can benefit you, your insurance company, and CareScout:

Providers participating in the CareScout Quality Network have agreed to preferred pricing on their rates. This preferred pricing can save you money on the services that you pay for yourself that are not covered by your policy. They can also save your insurance company money on services covered under your policy, which, in turn, can help your policy benefits last longer. CareScout will also be compensated by your insurance company if you choose a Provider in the CareScout Quality Network. Keep in mind that Providers may from time to time adjust their rates to which preferred pricing apply.

Preferred pricing may not be available in all instances:

Preferred pricing may not be available in all instances. For example, preferred pricing will not apply to services paid by a federal or state health care program. Also, preferred pricing may only last as long as the provider continues to participate in the CareScout Quality Network. In addition, preferred pricing may only be available to new customers of a Provider in the CareScout Quality Network and not existing customers of that Provider. Please call CareScout if you are an existing customer of a Provider and would like to know if it is part of the CareScout Quality Network and, if so, whether it will provide you with preferred pricing.

 

CareScout’s Quality Standards:

Enrollment in the CareScout Quality Network means that participating Providers have been reviewed against quality standards for licensure, certification, accreditation, financial health and organizational integrity. CareScout reviews these Providers through a rigorous process, including, but not limited to, evaluating the Provider’s organization structure, management and staffing procedures, organizational sanctions, operational procedures, cultural competency, and customer satisfaction reviews. As part of CareScout’s performance management program, these Providers are periodically monitored.

In addition to our rigorous credentialing process, CareScout confirms that Providers take a person-centered care approach with their customers. Person-centered care looks beyond safety to integrate a person's values, preferences, and goals into the care they receive. A person-centered care approach creates a partnership between caregiver and care recipient that addresses the physical, mental, spiritual, and social elements of a person's health. CareScout regularly surveys both a Provider’s customers, as well as their staff, to measure how well they adhere to the person-centered care approach.


Relationship between CareScout and Providers

CareScout and Providers have a contractual relationship that provides the CareScout Quality Network. It is important to note that CareScout is not paid by the Providers for the search results. However, CareScout may receive compensation from an insurance company or a member for any preferred pricing received by the member. Please be advised that this compensation does not affect the search results.

CareScout Field Team

If you are interested in becoming a CareScout Field Partner, you can contact CareScout on its website. A CareScout Field Partner is an independently contracted healthcare professional and does not have an employment relationship with CareScout. Contacting CareScout is not a guarantee you will be selected to be a CareScout Field Partner and is not a contract offer. The website does not provide all required steps to become a CareScout Field Partner and is subject to change.

YOU ACKNOWLEDGE AND AGREE THAT THE LIMITATIONS SET FORTH ABOVE ARE FUNDAMENTAL ELEMENTS OF THIS AGREEMENT AND THE SITE WOULD NOT BE PROVIDED TO YOU ABSENT SUCH LIMITATIONS. Indemnification

You agree to indemnify, defend and hold harmless CareScout and its affiliates, officers, directors, employees, and agents from and against any and all claims, liabilities, damages, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising from or related to your violation of this agreement, or obligation set forth herein. You agree to cooperate as fully as reasonably required in the defense of any such claim or action. CareScout reserves the right, at its own expense, to assume the exclusive defense and control of any matter subject to indemnification by you, in which event you will fully cooperate with CareScout in asserting any available defenses.

 

Proprietary Rights

All materials and content on this website (as well as the organization and layout of the website) are owned and copyrighted by CareScout or its Providers and may, except where expressly indicated, be accessed, downloaded or printed for your personal non-commercial use only.

In addition to copyright laws, you acknowledge that the content and materials on this website may also be protected by trademarks, patents, trade secrets, and other proprietary rights or laws. Without the prior written permission of CareScout , you may not frame or use framing techniques, sell, license, rent, modify, distribute, copy, reproduce, transmit, publicly display, publicly perform, publish, adapt, edit, or create derivative works from any material and content on this website, in whole or in part for your commercial use.
