"My father was put in this facility by my stepmother. While I visited my father who was bedridden, I noticed that the facility and room that he was in was very dirty even the screen was torn. My father was 95 and a half years old and was wearing diapers one of the days that I was there no nurse even came to change his diaper I had to go get them, nobody tried to feed him and nobody ever came into the room. There is no phones in the room or no call buttons. While I was there I saw a rat run across the floor to the opposite room that he was staying in. I think the nurses are lazy. While I was there I saw hospice only once. I was there for 4 days. Unfortunately my father passed away in that facility. I would not recommend this facility to anybody." - June… Read more

"I wanted to commend the operations at Emerald Shores. It started with Amy in admissions personally meeting me at the hospital concerning my mother's medical issues. This includes the inability for her to take care of herself at home. Amy was nothing but kind, supportive, listened and promptly had her cleared for admissions to her facility. Upon being admitted the nursing staff took great care of her from checking in on her, issuing meds, assisting her when she needed to get up, consistently take her vitals, listening to what she needed, and treated her like family. When questions came up beyond what the nurses could answer top administrative staff personally met with me, listened and resolved any concerns. They also welcomed any further contact or questions. "Orange park Rehab and nursing center is just ok on the care side. The food leaves something to be desired, besides being served cold and not what someone asks for from the approved menu the food is not one of their concerns even though it is a factor in the persons level of well being. The the rehab itself is very adequate. The therapists are wonderful. As for personal care it is a mixed bag. Some pts are very good but a few could ruin the review.
